I the undersigned beg most respectfully to state you the following few lines for your kind information and appropriate action.

1. That sir, I have opened a PLI through your good organization bearing Policy No – APS538665L
for the amount of Rs. 635/- per month is deducted from salary.

2. Even after 10 yrs, subscription of premium. I am withdrawal of Rs 52000/ through the loan from the concerned policy and Original PLI Bond Certificate submit to your Cell in 2014. In the month of Jan 2015 I am deposit the Loan amount with interest but the Original PLI documents are not receved till date.

3. It is therefore, requested that please fwd the original Bond certificate of PLI bearing Policy No – APS538665L in r/o Nb Sub Akhatar.
