This has reference to my letter dated oct.11, 2019 regarding correction in the death/ birth certificate as mentioned above (Copies enclosed).
It is a matter of regret that despite writing to you almost one month back no correction action has been taken so far. I once again request to kindly take necessary action in correcting my name as pawan and not pawan kumar as wrongly written in the certificate...
