Central Bank Of India — 15g form submitted but tds deducted

With due respect this is to inform you that i have submitted my 15g form on 20/04/2015 for the assessment year[protected] but my tds has been deducted. Due to your central bank of india, one of your staff, of hari mohan ghosh road branch has not entered my 15g form in your system. Now, sir i request you to reverse my deducted amount form my fixed deposit to my sb a/c [protected] (Central bank of india, hari mohan ghose branch),
