Airtel - SIM Card — Registered on some other name

My sim card has heen failed when i went to costumer service station to bring new sim they informed me that sim is not registered on my name whereas i submitted my own documents to the dealer on 2-2-11 and i am using this since last 6 months please please solve this problem as early as possible .
