Delay in settlement

This is to inform you that Ms.G.Lakshmi Manasa d/o G.Yadagiri aged 13 months was admitted in Hospital on 26/01/2012 at 8.00AM in Lakshmi Health Care Hospital ,c-84,Near Complex,Vanasthalipuram,Hyderabad - 500070 for treatment of Cough and Fever.

Further we would like to inform you that we have applied for Health Insurance card of the above employee on Dec 16,2011 but we have not received yet.
