Penalty paid: Rs.5000.00 vide receipt dated 16.10.2009

Further to my passport application transferred from Trichy RPO to Coimbatore RPO, I have re-applied for my passport on 23.10.2009 at Coimbatore Passport office and my police verification against this application completed on 11.11.2009 from Avinashi Police station. Recently I have checked my passport status in the online and noticed following status and finally asked for original Ration card / Electoral Identity Card.

Online Passport status noticed:
1) on 05.12.2009
Police Report is clear and the passport is expected to be dispatched by[protected]. Subject to all documents being in order.
2) on 09.12.2009
It is to regret that due to unavoidable circumstances the dispatch of passport has been delayed and will now be dispatched in a week's time.
3) on 22.12.2009
Original/Attested copy (by a gazetted officer) of S.S.L.C/School Certificate /Birth Certificate. Original/Attested Copy (by a gazetted officer) of Ration Card /Electoral Identity Card/ Voters List.
Report to this office on any working day (except Wednesday) between 9.30 A.M.
and 12.30 P.M. alongwith the above mentioned documents / this letter.

But, I have the Ration card and Electoral Identity Card in my native place address at Nagapattinam and those proofs already submitted with application at Trichy passport office and the relevant police verification already completed on 02.10.2009, later my file was transferred to Coimbatore as my present resident and working location is fall under Coimbatore RPO. So, I have submitted following available proofs at my present address during submission of my passport application at Coimbatore passport office.

1) Certificate from Employer of reputed companies on letter head
2) Statement of running bank account
3) Gas connection Bill
4) PAN card/Income Tax Assessment Order
5) Rental deed

Initially I had applied passport at Trichy Passport office vide Application No. TRY A04282509 on 18.09.2009 because I have Ration card and Electoral Identity Card in my below native address which fall under Trichy passport office.

My police verification at the above native address completed on 02.10.2009 and later I had met Smt.Gowri Balakrishnan Superintendent at Trichy Passport Office regarding delay of passport. Madam said that the police report says I am presently working in Coimbatore and she asked me to pay penalty of Rs.5000.00 for not mentioning present resident/working address at Coimbatore district. I had explained, by mistake I am not written that because I have the ration card & voter identity at the above native address and hence applied for passport at Trichy RPO, after their explanation I had realised my mistake that I must mention the present address also and the same day I had paid penalty Rs.5000.00 vide receipt dated 16.10.2009. After that madam informed me that my file will be transferred to Coimbatore RPO for the issue of passport because my present address is falling under Coimbatore RPO jurisdiction. Also madam asked me to contact Shri.B. Balamurugan at Passport Officer, Coimbatore and madam assured that I can get the passport with proof of certificate from the present employer, bank account and Gas connection bill at present address.

After two days, I received letter from Trichy Passport office informing that my file was transferred to Coimbatore passport office for the issue of passport vide their Letter No. TRYA04282509 dated 19.10.2009 and instructed me contact Coimbatore passport office. After two days I came to Coimbatore passport office and enquired with Assistant Passport Officer Shri.C.G. Rajan and they immediately found my application file transferred from Trichy RPO and asked me to submit fresh application mentioning present. Same day I have submitted new application and my application was accepted vide ref No.CBE A02953709 DT.23.10.2009 with following available address proofs.

1) Certificate from Employer of reputed companies on letter head
2) Statement of running bank account
3) Gas connection Bill
4) PAN card/Income Tax Assessment Order
5) Rental deed

Sir, I am not having Ration Card and Electoral Identity Card at my present address and humbly requesting you to accept above proofs and release my Passport at your earliest.
