Dear Sir/Madam,
My card no.is YHG0562801
There are three wrong entries. Pl. correct them and send the new card to the Adrress registered.
1.my birth year is 1990 incorrect
It is to be corrected as 02/02/1989.
2. Father's Name printed is 'maneesh akalesh'
Pl. correct it as'manish akhilesh'
Pl. send the card after correction to this address.
