Subject: Difference in Payee’s Name in PF Amount Cheques
PF Account No: AP / HY / 56966 / 03770
Dear Sir /Madam
This is to inform you that Employee Provident Fund Organisation, Regional Office had issued the Cheques of SBI for my PF amount settlement to the ICICI Bank, Rajkot where my Saving A/c is there.
Now the ICICI bank has returned the cheques to me at my residence address as cheques are issued in the name of “KANJIBHAI SHINGADIYA” in stead of “ROHIT SHINGADIYA”.
The printing mistake is either done by ICFAI or Employee Provident Fund Organisation. I had applied for my PF amount in the month of September 2008. I was continuously following this case and finally after 8 months I have received cheques with difference in payee's name which has frustrated me.
I kindly request your good self to please reissue the cheques in the name of Mr. Rohit Shingadiya with ICICI Bank Account No 015301514557
Further I would be eligible for the Interest of 8.50% on my total PF amount for whatever delay period has occurred.
I request all of you to provide your kind co-operation to bring the immediate solution of above matter.

I have filldup the withdraw form for my PF deposit last three month back and documents deposit to my previous organisation m/s GATI LTD. Till now I have not received my PF amount. Please confirm my status.

My PF Account No./ is AP/HY/56966/01492. Ihave inquired about my PF Account balance to The ICFAI Academy, the employer, but they have not yet responded since last 6 months.

I was enquired from PF office from Rajkot They Said minimum five years required to take advance. Sir I have already borrowed from my friend I have to return to him (Rs One Lac) . Kindly do needful for me.

Dear sir,
I have deposited my final claim in the month of April 2009 and due to some observations with the claim it has been returned back and the same was re-submitted to your department in the first week of july 2008. But till date the same has been not received by me or not credited in the bank as per the details mentioned in the application for final claim. You are therefore requested to look in to the matter and do the needful at the earliest. The details of my PF A/C No is. AP/HYD/42142/037.
