Employees Provident Fund — Provident Fund not received

I had surrendered my form for my Provident Fund claim in June 2010, I was told that would receive the amount within 6 months to my bank account, ( bank account details given ). Now 11 months to 1 year is over and I have not yet received my dues. Kindly check and let me know, details given below.
