[Resolved]  Employees Provident Fund — Subscriber name wrongly printed on provident fund slip issued for[protected] &[protected] PF Account no:(10403)OR/BBS/0003100/000/0001211

Dear Sir,

I Sri Ajit Kumar Ojha S/O-Gangadhar ojha At/Po-Puruna Balasore, Dist-Balasore,mob-[protected] working in accounts department of Balasore Alloys Limited .My provident funds account no is (10403) OR/BBS/0003100/000/0001211.
I have received my provident fund slip for the period[protected].I found that my name has been wrongly printed as AJIT KUMAR DAS in stead of AJIT KUMAR OJHA. Last year the same mistake occurred in my provident fund slip and same had been communicated to E.P.F. Office(BBSR) through our administration Dept.of Balasore Alloys Ltd. for correction.
But till 28.03.2012 it has not been corrected .

There fore I pray that , Kindly Correct my name in Provident funds slip Ajit Kumar Ojha , So that I will be remained grateful to you .Your kind co-operation in this regard is solicited.
