Dear Sir,

Myself Gurvinder Singh Rehal, i was working with M/s. ICICI Lombard Gen. Ins. Co. Ltd. three years (Jan 2007 to Jan 2010). I resigned from ICICI Lombard in Jan 2010 and i applied for my EPF in May, 2010. but till date i have not revived my EPF claim amount. Last week a got a mail from EPFO Bangalore office, which was they stated that my claim was settled and the claim cheque was sent at my Bank branck where i hold my bank account but when i checked the account details where they sent my cheque were totally wrong (Bank A/c. No., Claim Amount, Bank Branch) all details were totally wrong and when i ask them on the mail about this mistake then they send me a Re-Authorisation Letter and told me that you have to apply again your EPF form.

On dated 25.11.2010 i sent all my necessary documents through Speed Post at Employees' Provident Fund Organisation, Regional Office 13, 'Raja Ram Mohan Roy Road', Bangalore-560025 but till date they have not reply me.

You are requsted please look into the matter and do the needful as soon as possible.
