To, The Branch Manager, ESIC-Branch Office Rajendra Place New Delhi My self Chandan Kumar Jha Esic no [protected] I have provide photo with family at 19/07/2011 by Raja Garden local office smart card camp but i have not receive my smart card till date 17/09/2011 so, I want know who he reply me about my smart card status hope also receive reply from your side. AS I APPLIED FOR PLASTIC CARD AND ALL FORMALTY DONE AND THAN I GOT THE MASSAGE THAT PLEASE COLLECT THE CARD FROM ASHOK VIHAR BUT WHEN I WENT THERE NOTHING, THEY HAVE JUST MISPLACED THE CARD
