non dematerialisation

my regd folio no. 18578 with 300 shares under three certificates numbers ; 16782/16783/16798

remain in physical form and could not be 'demat'.
All written requests for the same and also for the change of address have not been attended.Letters are returned. All correspondence is just hopeless.
