Dear Sir, I had applied a Duplicate PAN Card under Application No [protected] on 01-Nov-2011 have provided all the necessary document as below along with copy of my earlier PAN CARD which is lost the payment is already throw Online Facility till date have not received the same every time the reason mentioned is as below. List of documents submitted till date. (1.Ration Card 2.Mom's Pan Card 3. Fathers Death Certification 4. Passport in the name of Vinayak Dattaram Lotlikar which also includes fathers name.) Neither my PAN is issued nor my amount is refunded back i have finally raise a complaint request you to please help. 1) Document provided as proof of father's name does not have name of applicant as per application. - Submit proof of identity of applicant where father''s name in application exactly matches with father''s name in proof. 2) Proof of Identity does not have name of the applicant as per application. - Submit proof of identity of applicant where name in application exactly matches with name in proof. The name mentioned by you in your application is 'Vinayak Dattaram Lotilkar' wheras the Proof of Identity and address submitted by you contain the name as 'Vinayak Lotilkar'. As per directives of Income tax Department the name in the application form and the name in the supporting docuements should match exactly.
Death Certificate of father can not be considered as Proof Of Identity. Only documents mentioned in Rule 114(4) of Income Tax Rules 1962 can be considered as valid Proof of Identity and Address.
