I HAVE LOST ALL MY LIFE INSURENCE CERTIFICATES, CAN I GET DUPLICATE CERTIFICATES? WHAT IS PROCEDURE OF THE SAME AND TO CHANGE MY RESIDENTIAL ADDRESS.

my policy no.553500733 from jamshedpur but i pay instalment twice in a year march and september from varanasi. last time i paid on 14/09/2011. and it was showed next premium due on 03/2012 and i was paid again on 10/03/2012 and it was showed again next premin due on 03/2012. i mine to say next premium date was not foreworded.
