Sir,
I am holding an LIC Policy with following details for which monthly premium have been deducted from my salary and deposited upto the month of June, 2009:
Policy No. 192085790
Name: Phool Chand Sharma S/O Sh. Prabhu Dayal Sharma
Commencement date : 28.11.1994
Pl.an : Bima Kiran (T. No. 111)
Premium: Rs. 90.00 payable monthly
Service Branch : S018
On 1st July, 2009 I have transferred from office of the Indira Gandhi Nahar Board, Bhawani Singh Road, Jaipur to the office of Rajasthan Electricty Regulatory Commission (RERC), Vidyut Bhawan, Jyoti Nagar, Jaipur. In RERC, there is no facility of deduction of LIC Premium from salary and get deposited in LIC.
In the light of the above position, I want to change the mode of premium payment from monthly basis to yearly basis. On enquiring from LIC Helpline (1251) I was suggested to wait for November, 2009 (the month of commencement of this Policy) for changing the mode of payment.
It is therefore, requested that the mode of premium payment of my above referred policy may kindly be changed from monthly basis to yearly basis as well as from salary scheme to Online/Cheque deposit scheme. It is also requested to kindly indicate the yearly premium on this Policy and the premium amount outstanding from July, 2009 to October, 2009 may also be indicated so that I can deposit the premium online or through cheque.
