I Shri.K.K.KISHORE KUMAR Nominee hereby declare that I am the nominee/appointed on behalf of the minor/nominee of the deceased Shri.J.N.K.MURTI .I further declare that I am nominated to claim the above JEEVAN AKSHAY –VI POLICY AMOUNT with your LIC OF INDIA, CITY BRANCH- VIII, Behind NTR Stadium, Lower Tank Bund, SECUNDERABAD, HYDERABAD ANDHRA PRADESH by Shri.J.N.K.MURTI deceased. The deposit monies held in safe custody of LIC POLICY NO:[protected] of Shri.J.N.K.MURTI deceased.

I have already submitted all relevant certificates including relevant Death Certificate of my father Shri.J.N.K.MURTI who expired on[protected] along with the copy of my Id proof and address proof with two witnesses .. It is requested that the full and final settlement of the deceased above policy amount of Rs 1,00,000/- alongwith final bonus was not settled so far even though I submitted the above claim on 28th October,2012 that is even after one and half month I did not receive the claim amount till date the branch have not any record of this policy what we can do better.

I therefore request your goodselves to arrange to settle the above amount at the earliest. I am very much worried whether the branch have any record of this policy or not.
