request for the return of refund receipt

Respected Madam,
I Mr. Yogesh .T. Bhanushali the client of your mediclaim policy no: 840313423 request you to grant me the refund receipt which I had submitted during the claim that I had taken during my hospitalisation for the refund my amount claimed. so please look after my request to send me the receipt as fast as possible.
