[Resolved]  NSDL TIN / PAN Card — PAN card not received

my name is meenakshi sharma.my father's name is rameshwar dyal sharma and my date of birth is 18-feb-1982.I have not recived in my pan card so plz send me a my pan card this my mailing address 14/22, radha villas, mathuradish ji ka mandir ke pass, patanpole, kota rajasthan-324009

I applied for the reprint of PAN card last year in Nov. I made the online payment but when I check through SMS it says it have been delivered on 24 of nov 2007 but when I called to customer care executive they told it has been delivered on 25 of Jan.
They are making clear about it. my pan no is BUAPS8907D.And my acknowledgment no-[protected].

I wanted to change my pan data for this i have applied my Acknowledgment Number -[protected]. i have check the status in nsdl web but status is showing there like-

Proof of address for communication not provided/ not valid
Proof of identity not provided/ not valid
so pls tell why is showing the status we have provided client declaration latte of id and address by gazzeted officer.
