I have lost my ATM card so please Block my ATM card As soon as possible.

I have lost my ATM card so please Block my ATM card As soon as possible.

Dear Sir,
I am VIJAYENDRA PRAKASH VALMIKI having SB A/C 01422041005537 with ANDHERI (W) BRANCH IN MUMBAI. I have lost my ATM CUM DEBIT CARD ON 03rd June 2012 at DADAR RAILWAY STATION. So please BLOCK MY ATM CUM DEBIT CARD and issue me the new one as soon as possible. Thank you very much.
