Paasport Office, Bhubaneshwar - PASSPORT NOT RECEIVED SINCE 2009

I am Deepak Kumar Sahoo staying at Rourkela.
I have not received my passport yet which was applied on 01-06-20009
at Bhubaneswar passport office.
I was called to the nearest police station for verification after 3 months of application.
The verification report was also sent by the police.
But I didn't get any information about it.
My application file no. is : BHU-A-015919-09.
