Loss of PAN Card

Dear Sir,
My name is Akanksha Srivastava PAN Card no. DBYPS6308R. I have lost my PAN card during a visit to a hospital in Bangalore, so kindly register a complaint against it so that no one can misuse that. Besides it I would request you to issue me a duplicate copy of that to the below mentioned same address.

My name is Sushma Garwadmath, PAN Card no. AUUPG9173C . I have lost my PAN card in travelling, so kindly register my complaint against it so that no one can missuse that. Besides, I would request you to issue me a duplicate copy of Pan Card to the below mentioned address.

For acquiring a Duplicate PAN card with existing details the applicant is required to submit a "Request for new PAN card or / and Changes or Correction in PAN data” form. This application can be made either online at our web-site www.tin-nsdl.com, or by submitting the physical form to any of the NSDL TIN-Facilitation Centres (list of which is available on our website) along with the necessary fees.
