personal particular form (for police report) wrongly sent to you attached with passport application form by post office bundi. Pl. retun my personal particular form to post office bundi or suptd. police bundi for police report.

When I see my status than (Less than 1 year stay; pls. send 3 personal particular forms for verification at each other address(es) during last one year and pay applicable penalty)
