Here is Akhilesh kumar Shakya, PhD student from Indian institute of technology Kanpur.I would like to inform you that Lucknow passport office issued me passport(H2994508)on the basis of application form
LKOA01131509/537/09 under tatkal scheme. But due to my unavailability at the time of police verification, passport office gave me notice for surrendering the passport immediately. I surrendered passport and
submitted affidavit for re-verification. Before 3 months police re-verification became completed but i did not get my passport.i need my passport so i am requesting you please do my help. I will be highly obliged you.

I have applied for online for passport.All the process i.e. Police verfication and LIU verfication is completed. Of late I have received the letter from Passport office dated 24.08.2009 (post office stamp 28.10.2009) that I have to given my high school certificate and confirm my current address. I have mailed the same on 30.10.2009 courier by First Flight Courier to the pasport office on 30.10.2009.( I have also acknowledgement of receipt of letter on 03.11.2009) I have regulary remind the office that I have Send the copy and attached the copy of all the document (for Reminder Purpose) But I have Not received the response from the passport office as yet.

Thank you very much for issuing me passport (No:G7592244 ),
which I received in May 2008. While going through its pages I noticed that my
date of birth and the spelling of father’s last name is not correct. Please note the
following corrections and make necessary amendments in my passport.
1. My actual date of birth is SEPTEMBER 22, 1988 but in the passport it is
wrongly printed as August 22, 1988.
2. The correct spelling of father’s last name in TRIPATHI but in passport it is misprinted
as TRIPAHTI.
Please also note that while filling the application form I wrote my correct date of
birth and other details.
I am sending you my passport back along with a copy of my high school
certificate (as a further proof of correct date of birth) with this letter. Please make
necessary changes in the system and issue me the passport with correct date of
birth and father’s last name.i was curirrier the pasport to the lucknow passport office but i have no replied any information from the lucknow passport office.i sended it before nine th monthi do not know that the passport reached there or not.so please kindly give me reaply for this matter.

Sir i had applied for my passport for urgent conditions. I also recived my passport that was issued on 24 april 2009 in time and also Local Police has done all veryfication as per their procesure. But I got no information through Lieu side also Lieu had not came any time to my home for any kind of verification and they prepared a wrong report that aplicant was not avilavle at the time of verification.
Then i got a lettr from Lucknow Passport office that pl surrender your self with passport immedeatly and i also surrendred mypassport with two PP form. Its gone more than one & half months but i have got no information about my passport. Pl try to issue my passport as soon as possible because sir as I aman Mechanical Engineer within last 2 months i have got more than 4 offers from MNC's for Gulf countries and not able to go and its affecting my carrier also.
