Passport office Patna - delay issuing in passport

sir,
I myself Prashant kumar applied for a passport,dated on 05-03-2011 bearing File Ref. No.-W370599-11. In regarding this i had submitted the following documents which are, (given, police verification completed)

1. Residential Certificate issued by S.D.O Muzaffarpur ---for Add. proof
2. Pan card along with letter that already contain residence add.
3. Learning driving license
4. Matriculation certificate ----for Age Proof
5. 10+2 Certificate
After furnishing all the regarding information, there still needed the furnish the required information within 15 days. When is checked my application status.
so, i have a humble request to please look into this matter and resolve my problem so that i could get my passport on due time. I shall be highly obliged to you for this kind act.

Grievance:
Dear Sir,
I have applied for the passport in the month of NOV 2011 but till now I haven't received my passport & not even the proper status on the Internet.Although police verification was completed already (in the mid of December).
On the Internet the only status written is "It is requested to furnish the required information within 15 days" from last 4-5 months.Kindly provide the proper status on the prior basis .Kindly update the status on Internet & if possible
please send the status on my E-MAIL id or via SMS which is mentioned below.
