SUB: Non Disbursement of Pension sanctioned to me under EPS-95, PPO No. 1021-reg.

This is to inform you that my pension payment has been transferred to the HDFC Bank, Sarjapura Road Branch, Bangalore, in 2006 and I had been getting the same till Dec 2009. However, this has been discontinued from Jan 2010, inspite of having submitted my life certificate to HDFC Bank, Sarjapura Road Branch, Bangalore in Nov 2009.

A copy of my account statement is attached fyr. You would note in the statement that the last pension of Rs. 841/- was credited on 02/01/10 for Dec 2009 and there is also a credit of Rs. 841/- on 07/04/10 for Aug 2006..
Request you to look into the above and continue my pension disbursement with arrears wef Jan 2010.
