I have received Rs. 122620/- through cheque no. 016044 dt. 12.12.11 against our claim of Rs. 151684/-.

Again I have sent claim of my wife Smt. Vijay Garg for the same treatment on 22nd December, 2011 to you alongwith original Bill No. 12037504 dt.12/12/2011 of Medanta Hospital, Gurgaon for payment and all other relevant documents in original. But till today we have not received any reply from you. Kindly let us know the status of my claim.

We, therefore, request you to please arrange to send the payment at the earliest through a/c payee cheque in the name of Mrs. Vijay Garg.
