Name missing on my voter id card

My self is Md Asif Iqbal and me name is wrong on my VOTER ID CARD as well my father's name. Thats why i want to improved my name. So please help me how to do it.

my name is praveen p s, but it is printed as praveen only, so it is making me more trouble
