[Resolved]  Voter Identity Card — surname,date of birth &address not correct

hi, sir this is mallesh arige i got voter id card but u people entered wrong address, i have face the lot of problems, so solved my problem, because of passport application is delayed that one.my card no is ZTS0160143.

Dear sir, I got my mother voter id in 9th aug 2007, but some mistake shown on my mother voter card .my mother original date of birth is 10/12/1963 where shown on card xx/xx/1967.my mother card no is -MLN1562941. address- kona purba para50 liluahhowrah-711114.my email id is [protected]@hotmail.com. & my contact no is- [protected].

I am Chiranjeevi Gudisi. my voter id number is XJB0271619.
But till now i am not get any voter id card and searching by name in ceoandhra.nic.in it will show chiranjeevi gudise. surname is not correct.
So how to modify surname and how to get my card.
