[Resolved]  Voters ID Card — NOT ACCEPTING MY FORM

i m staying in vasco last nine years.i have tried to summit FORM-6 in marmagao municipal vasco for voters id card for two times but they have rejected my form say that it is incomplete.
their is a Particulars(III) where it has been ask:

III. Details of member(s) of applicant’s family already included in the current electoral roll of the Constituency:

But on one in my family hold voter card so i keep it blank.
and they reject it say it is incomplete and also don't me tell what should be done for it.
can you tell me what to be filled if on one in family has voter card????
