DHL — Non delivery of shipment

Hi, I have booked a courier through DHL for international delivery from Chennai to San Jose, CA on 14th August. The booking was made through a local agent in Mylapore. Its now 10 days and still the shipment has not been delivered. And on enquiry, DHL says that they dont have address information. And the agent says that we have given all the information.
