Name - Anil Kumar IP no.- [protected] Dispensary - Okhla phase - 3, Modi mil (New Delhi) Incidence.- Due to emergency i was admitted in Hospital Sanjivani (Khasra no. 284, Pocket - 5, Jasola New delhi - 110025) on the dated of[protected] and discharge date was[protected]. I had submitted my all documents to Okhla dispensary in oct 2014. After that i had submitted 3 applications into dispensary dates are[protected]/22-07-15/15-10-15. No actions is taken till date and dispensary employees doesn't give any update on the same. You are requested kindly help me out to reimburse the same Claim amount is Rs-11732/-
