ESIC — Query about cancellation of cashless facility to my mother Mrs.Kokilaben V. Limbadiya

Dear sir My name is Vishal V.Limbadiya, my ESI insurance no. [protected], My ESIC Date of Entry is 05/05/2006 and Date of Issue is 22/06/2007 My mother Mrs.Kokilaben V.Limbadiya is started treatment in ESIC Model Hospital, Bapunagar since July'2007 and there they fond critical deices in kidney so they send us to Ahmedabad Civil Hospital for further treatment in duration of 10/07/2009 to 25/12/2009 Second time my mother was admitted in ESIC Model Hospital on 27/05/2012 from there they send us to Apollo Hospital since then to now my mother is having cashless treatment under Super Specialty Services But today I came to know that my mother's cashless treatment under Super Specialty Service is cancelled because of some issue in Date of Registration Medical Officer of Model Hospital Bapunagar tell us to contact you at Delhi H.O. by mail/phone or Visit I tried to contact on these number[protected], [protected],[protected],[protected] but no one is receiving call so I am sending you this mail and some documents regarding registration and treatment you can find in attachments I hope you refer it and do further process to continue treatment Please find attachments Kindly reply soon Thanking you Vishal Limbadiya [protected]
