[Resolved]  ESIC — Rebate amount not received

Dear Sir,

I am "PATEL ANILKUMAR AMBALAL" holding Employee's State Insurance Corporation (ESIC) IP No. [protected].

I have put my rebate claim on 10/08/2013 at D-2 Dispensary (Juna Vadaj, Ahmedabad) of Rs. 82536 for Emergency Treatment taken for my wife during 26/07/2013 to 06/08/2013.

I have allready solved queries many time.

ESIC Slogan is "IP - VIP" but I haven't received the said rebate amount till date, So I request to you to do the needful as early as possible.
