ICICI Bank — Not yet received credit card add on request

hi, i applied My icici add on card request for past two month ago, reg these i contact customer service also two times, but still no body can revert properly, i submit all docs and i have acknowledge copy, pls check and revert, here enclosed to my acknowledge scan copy.
