With reference to the above policies, I have already sent cheque no.647469 & 647470 of Axis Bank Ltd at your Badakdev Branch at Ahemdabad, Gujarat on 2.07.2015 But your department have received the same and returning the same with out depositing the same, in this way my policies will be discontinued and ICICI Lombard will be solely responsible for this type of discontinuation of helth policies.Even though i have not received hard copies of any notices.
