Respected sir, I am a holder of connection by consumer no. 004915, our distributor is DANKUNI GAS DISTRIBUTOR, dist. Hooghly. My name in the connection card is mentioned as GAURANGA SEKHAR DAS, but the correct spelling of my name is GOURANGA SEKHAR DAS. The addhar card has my name as GOURANGA SEKHAR DAS. I contacted the distributor regarding the correction in name. He told me to submit a affidavit on stamp paper and other form as provided. I request you to change my name as mention above and direct the distributor to make the changes. To avail subsidy I request you to kindly change my name as "GOURANGA SEKHAR DAS" in your record.
