My name is Brajakishore DashMohaptra.The above name correctly mentioned in your Origianl receipt i.e. SV re ceipt bears No.718198 dt.14.01.04.But in the time of Computer feeding my name has been wrongly mentioned as BRAJA MOHAPATRA.Hence I am requested somaney times for correction my name from Braja Mohaptra to BRAJAKISHORE DASHMOHAPATRA and submitted all documents i.e. EPIC, ADHAR CARD, BANK PASS BOOK, SV RECEIPTS etc.But nobodey can respond my request.Lastly I am to request to you kindly take necessary steps to correct my name or send the name and email address or phone Number who will be take step.
