My house lpg connection has been dew deactivated. i have register complain (no. 2694836) on 13.07.2016.
I am submitting the following documents —
1) post retirement medical card indicating employee code (71016), date of birth (03.02.1952) & house address (akashdeep, plot no.-22, kallol co-opt. housing society, brajanthchak, p. o-haldia township, purbamedinipur,
Pin-721605)
2) present id including contractor employees gate pass (no.19-jul-16/labour/115362/509036)

Please reactivate my lpg indane connection at my house address.

my complain no. is 2694836 dated 13.07.2016 for reactivation of my lpg indane connection no.21945. post retirement i have taken up temporary jobs as deputy general manager in iotl, boot-3 of paradip refinery, odisha. iam presently staying in rented house and manage my cooking by lpg from local markets.i have not transfer lpg connection to paradip, odisha due to temporary nature of jobs.my own lpg connection is at my permanent residential address . recently it has been deactivated.

request you to reactivate.
