My son Vineet Agrawal drawn a single Premium Policy for Rs. 30000/- from LIC, Salt Lake Branch, CF 335, Sector - 1, Salt lake City Kolkata - 700064 on 09.02.2011. All of a sudden he died on 22.11.2013.
I as a nominee visited the LIC office, Sector - 1 on 07.01.2014 for Claim Forms and other formalities. After a long waiting, I received claim forms from the dealing official Mrs. Madhuri Mukherjee. There were so many formalities.
I again visited the office with all the documents like Death Cert., Original Policy Bond, Original claim form completely filled in including leave record of the deceased, Suppl.Post Mortem Report, Xerox copy of my Pass Book and other supporting documents on 12.06.2015. Again I could not meet the same dealing official that's why I met the Branch Manager and requested him to receive the forms. Branch Manager checked all the documents and verified with original and asked me to deposit the same on the receiving counter. I deposited the forms and in turn received a receipt on form no. KSDO/OS/118.
I received a letter dated 13.06.2015 from the office that form submitted by me was incomplete. I surprised to note that the BM advised me after proper scrutiny to deposit the form on the counter, then how the form was incomplete. For the reply, I written a letter to the Branch for apprising me details. Again I received a letter dated 25.06.2015 from the office without explaining my queries raised in my letter dated 20.06.15.
I am resident of Patna and it is not possible for me to visit office regularly. I think that office is harassing me and I need relief, that's why this complaint against the Dealing official and the Office.
