[Resolved]  Lok Nayak Hospital New delhi — Wrong Information on Birth Certificate

Sir/Madam, My wife delivered a baby in lok nayak hospital. All the information on the slip was correct and the information of discharge letter is also correct But they have sent the incomplete information about the mother and father name to the mcd office. Now when I go to the MRD office they demand to submit the copies of identiy proof for both the person and also an affidavit from SDM. You will find very hard if the information on the berth certificate be correct for a single person. Why we are responsible for any incorrect information made by the hospital. In all my documents, the information about my name and my wife name is correct but they have sent the name to MCD office without adding the surname. Why? Thanks
