I subhasish Banerjee vill;Ballavebati.po; munshirhat dist;Howrah, policy no;H043/[protected]/01, have admited west bank health and wellness institute, 120/1, andul road, Howrah711103 dated on 25/09/2014. I also have submitted claim form at LIC'S Domjur, howrah branch dated on 16/10/2014.But Ihave not received any information till now.It is injustice to me.If you kindly take the matter, I shall be highly obliged.
