Ltd Grievance Redressal Officer – 3, Middleton Street, KOLKATA - 700 071 Respected Sir, Re: Policy No 240100/48/14/[protected] Sub: Settlement of Claim This is with reference to rejection letter dated 22/09/2016 (Copy enclosed) received from National Insurance Company Ltd – for the captioned policy No 240100/48/14/[protected]. In this connection, I have been given to understand from TPA - Medsave Healthcarre (TPA) Ltd. that the claim is not settled as my policy has completed only four years. In this regard, I would like to bring to your notice that my wife Mrs Shobhana Nitin Kakaiya had no previous history of illness. This is the story of not 4 – 5 years but it is of 3-4 months only. During June – July 2015 she had back pain below neck. So we consulted our family doctor. He gave us medicines. This has happened repeatedly for 2-3 times. So our family doctor advise us to take out X-ray. Accordingly we had taken out x-ray. On examine the x-ray, he advised us to see another surgeon doctor for better treatment. As per his advise we consulted another Doctor who gave us medicines for 3 weeks & advised us to meet him with MRI report after 3 weeks. But unfortunately, just within one week my wife started pain on her shoulders & her hand-grip was becoming weak. Immediately we consulted the Doctor. Doctor advised us to take out MRI immediately. Accordingly we had taken out MRI. On examining MRI report he said my wife has to be operated. In this connection we had taken other two doctors opinion also. They also said us the same “operation is must”. Accordingly on 5th October, 2015 my wife had been admitted in Global Hospital for operation. On 6th & 7th she was operated. After one week of the operation she developed Neumonia in hospital itself. She suffered for almost one & half months and passed away on 11th November 2015. Heavy expenses (Around Rs 11 Lacs) was spent for the Medical treatment of my wife late Mrs Shobhana Nitin Kakaiya. I have borrowed lots of money from my relatives and friends to clear the hospital bills. So, there was not a past history for her illness, not more than six-months.
