Dear Sir, I filed my claim for Rs 23521/0 yourself was pleased to pass it only for rs 20921/- after deducting a sum of Rs 2600/-. As per the policy in the case of hospitalization a sum of Rs 500/- is payable to the attendant. I have been admitted in the hospital for 4 days, i.e 9.6.15 to 12.6.15. As per the terms of the policy we are entitled for a sum of Rs 2000/- as attendant allowance. Keeping in view of the above, I request you to reconsider my claim form and pay me the balance amount, or provide me the reason why you have deducted other wise I have to file the case in the consumer court.
