i have applied for pnb-oriental -royal mediclam policy schedule, and i got the policy number 224600/48/2016/2670, insured code no 69650253, issue office code 224600 issue office name DO BAREILLY, and insured Name is Smt Zinat, but the name of the dependent name is entered wrong in the policy paper and card, the, dependent name is wrongly entered as AKSHAY KUMARI MISHRA with is wrong name, so kindly do the needfull as soon as possible so i got the dependent name Zinat
