PNB — Wrong debit entries from my SF account

Address:Ludhiana, Punjab

From my above said account on 21/05/2014, above said amount has been Debited through ATM. Please let me know from where & how this amount has been debited. I may inform you that Atm card is with me and have not handed over my Atm any one.
Please guide me properly and provide me credit of said amount.
