Reliance Fresh — Cashback not received

Dear Sir/Madam,

I have purchase of Rs. 3500/- as on 25/01/2016 from your Anandnagar Store situated at prahladnagar main Road, Satellite, Ahmedabad-380015. I have paid the bill amount by SBI Debit Card. At that time the SBI Customer 5% cash Bank offer was applicable. but still i have not received any cash from your side. so please look into this matter and credit my cash back to my account.
