Reliance Gsm — Sim registration failed

My name is lucky agrawal.my number is [protected] c/o narendra agrawal reliance gsm postpaid. my old sim suddenly work stop. so i submit my old sim, and i get new sim on 27.08.15 after all documents submit. i insert the sim every our but i still get the message, sim registration failed. please solve my problem. my alternate number is [protected].
