i abhilash.mj would hereby like to inform that i had purchased a mobile phone policy no.421704/48/2015/695 vide invoice RTN-5366 dated 13/01/2015, bearing IMEI no.[protected] and Make micromax canvas a114 2.2 BLK . The said mobile phone has been stolen on 23/04/2015. Brief Description of incidence: I had gone to the xerox shop near Ambedkar medical college road opposite to Manjushree hostel, shampur, rtnagar 560032 and had placed my phone on the table and some culprit has taken away my phone without my knowledge. On calling my phone number [protected] after this incident the sim card has been removed from the respected phone conforming that this is a thieft. I have blocked the SIM card with the help of the TATA DOCOMO service provider immediately and informed the D.J.Halli police station, Bangalore city and have lodged a complaint and received a reciept from the police station as well conforming that the comptaint had been filed.
