Adecco — how to claim the pf amount from adecco trust

I was working with UPL Limited (Unit-5) My Empiloyee code was E15852 & UAN NO. [protected], my joining date was 02.05.2016 and date of releaving was 11.01.2018.

Kindly guide me how I can refund my Full PF amount.
